Applying systems-thinking to redesign a multi-specialty operation theatre - Findings from a full-scale simulation study

DescriptionOperation theatres (OTs) are dynamic, high-stakes workspaces where operational inefficiencies and equipment clutter pose potential risks for surgical error. This case study documents a 20-week human-centred design process to observe, ideate and redesign the multi-speciality OT at a public tertiary hospital in New Delhi, India. Beginning with a brief aimed at electrosurgical tools, field observations and stakeholder interviews revealed that tool usage and performance was inseparable from the broader human-environment-technology ecosystem. The design scope was consequentially expanded to address systemic inefficiencies across five interconnected equipment - the case trolley, surgeon’s seat, electrosurgical tools trolley, electrosurgical control unit, and the OT lights. Using a systems-thinking framework to synthesise observations into actionable design directions, the team developed full-scale mockups which were evaluated by surgeons and biomedical experts. Findings highlight the value of holistic, participatory design methodologies in healthcare environments and identify clear directions for future functional prototype development and scenario-based usability testing.
